I think the reason people are flipping out is two things.
One is the level of realism, as the one person reacting to the guy bending over, and the simple act of a guy having sex with is pregnant wife.
The other is the level of fetish-y UNrealism, in the size of the female's belly while walking around without any obvious effort, and how in the many sex scenes her stomach seems to just keep growing and growing, showing that the artist was either really turned on themselves by the idea, or trying hard to appeal to those who are.
The concept of a guy looking 'normal' and doing something 'normal' like having sex with his pregnant wife, while also being combined with niche fetish stuff *does* kinda ruin this for all but the least picky and most into the fetish, I think.
