I can't really read Japanese, but I can read the phonetic characters mostly, and I watched enough anime to pick up common phrases. I can tell the main character doesn't know what's going on. Things like "why is this..." as he is about to cum for the second time. That was pretty clear from the comic though.
I think the lines that look like "**(**)##" are "name (proununciation) Age". Can't read 'em tho. MC is 34, doc is 40.

Page 20 I think the doc says "it's done" and main character is sort of disagreeing tongue in cheek because he's still cumming even after the fingers are out. Then doc is cleaning his hand and saying in 2-3 (unit of time), which I'm inferring to be a followup appointment instruction.
At the end with the written instructions he's reading he seems to be questioning it but in a way where he's also questioning himself.
